The referral process for physicians, hospitals, and nursing facilities to Karemore Hospice begins with determining patient eligibility, which typically requires a prognosis of six months or less to live and a need for palliative care. Physicians can initiate referrals through phone, email, or referral forms, while hospitals and nursing facilities often utilize designated liaisons or social workers for coordination. Referring providers should gather relevant medical documentation and obtain consent from the patient or their family to share information.
Once the referral is submitted, Karemore Hospice will schedule a home visit for an initial assessment by a hospice nurse, which includes evaluating the patient’s medical, emotional, and spiritual needs. Following the assessment, an individualized care plan is developed by the interdisciplinary team in collaboration with the referring physician and family members, ensuring clear communication about the patient’s care.
Ongoing communication between Karemore Hospice and the referring provider allows for updates on the patient’s progress and collaborative decision-making. Should the patient’s condition improve or transition to a different care level, discharge planning is coordinated, and bereavement support is offered to the family after the patient’s passing, ensuring continuity of care and support throughout the process.
