Proof of Identity (i.e. Driver’s License, Valid Photo ID, etc.)
Social Security card
Certification (+skills), California License RN/HHA/LPN/PT/OT/SP
CPR & First Aid
TB (PPD) Screening Within six (6) months of application/Chest X-Ray (Valid for 5 Years)
Physical Clearance
Background Check All Staff will need FBI and Social Services Background checks.
HEP B Vaccine/ COVID Vaccines / Declination Form
