Hospice care is available to patients who:

  • Have a terminal illness with a life expectancy of six months or less, as certified by a doctor.
  • Are no longer pursuing curative treatment, choosing instead to focus on comfort and quality of life.
  • Have the consent of a physician and agree to receive hospice care.

Patients of any age, suffering from various conditions such as cancer, heart disease, lung disease, dementia, or other terminal illnesses, may be eligible.

Hospice care is typically covered by:

  • Medicare: Medicare Part A covers hospice services if patients meet eligibility criteria.
  • Medicaid: State-specific Medicaid plans often include hospice benefits.
  • Private Insurance: Many private health insurance policies provide hospice benefits. The specifics depend on the insurer, but most policies cover hospice care similar to Medicare.
  • Charitable Care: At Karemore Hospice, we work with patients and families who may need assistance in case they lack adequate coverage.

If desired, Karemore’s hospice physician can take the lead while continuing to consult with your primary doctor. Communication and coordination are key to ensuring the best possible care.

Initial Assessment

 Care Plan Development

  • Individualized Care Plan: The interdisciplinary team develops a tailored care plan based on the assessment, with clearly defined goals.

Communication

  • Family Meetings: Regular meetings to discuss care goals and progress.
  • Ongoing Updates: Continuous communication for addressing concerns.

Implementation of Care

  • Coordination of Services: Medical care, pain management, and emotional support provided as needed.
  • Emotional and Spiritual Support: Counseling services for patients and families.

Ongoing Support

As a hospice patient, you have several rights, including:

  • The right to be fully informed of your care plan, treatment options, and changes in your condition.
  • The right to accept or refuse any treatments.
  • The right to have your privacy and confidentiality protected, especially regarding your personal health information (PHI).
  • The right to receive care that prioritizes your comfort, dignity, and preferences.
  • The right to receive services free from discrimination based on race, ethnicity, gender, age, or any other factor.

At Karemore Hospice, we ensure that all patients receive the highest standard of care and respect for their rights.

Hospice care is designed for patients with a terminal illness and a life expectancy of six months or less. However, patients can continue to receive care as long as their physician certifies that their condition remains terminal. If a patient’s condition improves or stabilizes, hospice care can be discontinued, and they can be re-admitted if their condition worsens later. Our team reassesses patient eligibility periodically to ensure continued care if needed.

If a hospice patient’s condition improves significantly, to the point where curative treatment or extended life expectancy becomes an option, the patient may be discharged from hospice care. Discharge does not mean that care is over—it simply means that hospice services are no longer required. Patients who improve but later experience a decline in health can always re-enroll in hospice care. Karemore Hospice will support you through any transition and ensure a smooth change in your care plan.

Yes! Karemore Hospice provides hospice care in the comfort of your own home, a nursing facility, or another long-term care setting, depending on your preferences. Our team of nurses, aides, social workers, and chaplains can visit regularly to ensure comfort and support, without the need to move into a healthcare facility. In-home hospice care allows patients to remain in familiar surroundings, which often contributes to emotional well-being.
